In black basalte, it shows a three-quearter profile of a man wearing a full fur cap. His hair extends below the cap to his shoulder. He wears a waistcoat and coat, with a stock wrapped around his neck but no ends visible at his throat. Below the left shoulder is the signature "Bentley," though the name does not refer to Wedgwood's deceased partner, Thomas Bentley.
The plaque is rectangular, with all four corners cut away to create an ocatagon.
